I just watched Pi (1998) (http://imdb.com/title/tt0138704/) and I am not saying it is the most disturbing, but it does have it's disturbing moments...in an odd way.

It must be realistic. When the movie ends, and if you look back into it and realize it is possible, then it is a great movie. Of course there are other elements that one should focus on also.
