Parallel worlds, father-daughter story.

Can anyone identify this movie?
There are couple of simultaneous story lines. Daughter's soul is stolen but daughter is in the hospital. Father rushed to hospital but makes accident. Father makes suicide and becomes memory-less and actually is the one who captures daughter's soul. There is blind guy who can influence our world from parallel by creating sequence of events. Evil guys with mirror type of face. Female character who tries to make father remember again and dies at the end.
