"Niku daruma" aka "Tumbling Doll of Flesh", a Japanese "pornographic horror" film... lets just say a lot of sick mutilations.

Those Japanese movies are crazy but so over the top that I get sick of them. For me it's probably The Human Centipede 2. I actually didn't think Wolf Creek was that bad for gore.

A favorite of mine....
I've watched numerous films lately on Fearnet that would qualify for this, and I've tabbed them, but without a crib sheet I couldn't name most, and you wouldn't care to see them anyway.

One I love that most dont like is "Salo" directed by Pier Paolo Pasolini. It is an extreme "art house" film. I know many of people who turned it off near the beginning of the film. My favorite scene is at the end, when all the "4 wealthy men" are watching what is going on in the courtyard...

I forget how gross A Serbian Film is because I really liked it. The Human Centipede is really only gross in it's idea; it doesn't compare to part 2. The Evil Dead remake I just hated. Dead Alive was a good pick, although it's comical. Salo sure is up there too.

Salo is just laughable to me. I didn't find it disturbing at all, just silly. Really silly.
The book however is ****ing disturbing (especially when they take out the veins of one of the teens and stick them into the ground while he's alive. Or something like that. )

This one actually made gore scary, which I'm not usually scared by gore, but the whole mood and atmosphere and it's EXTREMELY GRAPHIC. Like it would break the MPAA (pervasive sequences of graphic horror violence, blood, and gore, disturbing images, some nudity)
It's crazy and I love it.

I can handle graphic and disgusting gore but when it comes to a simple up close shot of a nail been pulled off a finger or a tooth pulled out, urgh. Oh, also death by a hypodermic needle especially if you see it enter the skin up close.
So any scene with the above in will freak me out.

I was more awed by than grossed out by Dead Alive. Its cartoonish comical characters and their antics made it easy for me to sit back and appreciate the crazy inventiveness of its splatstick.
But a film I felt genuinely deeply disgusted by was Visitor Q, a disquieting and bizarre little horror film shot in verite style. It's really quite repulsive.... not merely superficially, kinda hard to explain why... It's not excessively violent compared to many horrors, indeed it's even slow-paced and reserved in a sense... but there's always this strange tone to the film, a very subtle sinister nihilism, so when the film does show its depravities they're that much more offputting because of it. It's seriously one of those few films that I think could genuinely do some psychological harm to unwary sensitive viewers.

Blind Beast I saw recently felt more unsettling and disturbing than Salo, Srpski Film or The Pig F*cking Movie.

Remember that quick image in THE RING (remake) where you see a nail graphically go through a finger. That image has always stuck in my head, and whenever I watch the film I always prepare myself. But since it's subliminal I never know when its going to happen so I can't avoid it.

ARGHHHHHHHHHHHHHHHHHHHHHHHHH. I looked! Yes, definitely remember that bit.
As far as mainstream films go I'd say Hostel - I'm sure there's gorier stuff out there if you delve into underground horror films but I'm not a huge horror buff, and Hostel was believable enough to be terrifying.
Punisher: War Zone is also pretty out there for a non-horror flick (it's basically the equivalent of a slasher film where the hero is the killer). Not a very good film but it'll definitely satisfy your lust for violence.
"Niku daruma" aka "Tumbling Doll of Flesh", a Japanese "pornographic horror" film... lets just say a lot of sick mutilations.

I've also seen "Flowers of Flesh and Blood" (Japenese indie horror flick) on Youtube, and it freaked me out when I was a teenager (might be worse than Hostel). But I haven't seen it in years so I'd have to watch it again to see if it's as scary as I remember.

I agree - not a huge anime guy myself but I've watched various clips of violent animes on the net, and most of them are so over the top that it's not "scary". To me realistic violence is a lot more terrifying than this "South Park" style violence since it's actually believable.
And there's also this - haven't watched it in full yet but it looks pretty over-the-top gorey too:

There is an Asian film called "Shadows in the Palace". Two scenes that stuck out was "sewing needles" underneath the finger nails... and self mutilation with using gold thread sewn into the skin. Tried to search for pictures but was not successful.
Blind Beast I saw recently felt more unsettling and disturbing than Salo, Srpski Film or The Pig F*cking Movie.
Also as a rule, sex doesn't disturb me in a horror film; I think one of the reasons that sex is considered "horror film material" to start with is because of the West's somewhat sexually repressed attitudes even today (likely due to the influence of Judeo-Christian cultural values). Think about it - showing sadistic violence and torture in a film like Hostel will land it an R rating, but just showing two people knocking boots uncensored would get it an instant NC-17 - that really says a lot.
That's one reason I hated "A Serbian Film" - it felt like a low-budget porno rather than a horror film (with the only somewhat disturbing part being about 5 minutes total depicting child rape). To me graphic violence and torture is way more frightening than two people humping, or someone with a kinky fetish- that's just me.

Blind Beast is kinky, but it's getting out of hand in the end.
And why the hell do American people care about all these R or NC-17 ratings? Isn't it like PEGI for computer games? Here in Poland nobody gives a flying damn about this PEGI stuff and I don't even think there are any ratings for movies.
Poland is Kinky Freedomland.

I don't - I was pointing out how an uncensored sex scene would get a film an instant NC-17, yet graphic torture could still get buy with just an R - and how stupid that is - it's just a remnant of sexual repression from cultures with a strong background in Abrahamic religions (Christianity, Islam Judaism, etc) - not to turn this thread political or religious, but it's worth pointing out.
